Domiciliary Support
Cardiff Mind is able to provide domiciliary support services for people aged 18 and over to assist individuals to develop and/or maintain their independent living skills and wherever possible, to secure substantive and safe accommodation.
Cardiff Mind is registered with CIW to provide a basic personal care element as well as general support if this is required in the support package.
Criteria for Support
The people that we support will have experienced some degree of mental health difficulty, and may also have support needs identified around issues such as substance misuse or criminal justice convictions.
What Support Does This Service Offer?
Cardiff Mind provides support to help tenants to live successfully in their community. Support services are provided to individuals and families whose inability to maintain their tenancy is putting their access to quality, safe accommodation at risk.
The support we provide may include some or all of the following, as well as accessing appropriate support from other agencies / organisations and this includes:
- Support can be provided to attend medical appointments and accessing medication
- Budgeting, maximising benefit claims, dealing with correspondence etc.
- Emotional support
- Assistance in attending any general/mental health appointments
- Accessing services which may promote development of social skills
- Accessing training, volunteering or employment opportunities
- Any tenancy related issues which have an impact on an individual’s housing and support needs
- Networking and advocacy issues
- Managing aspects of mental health difficulties
- Support to contact other services
How Do I Apply for Support?
A number of areas can be identified on a care plan via a unified assessment. This support can be flexibly arranged with each individual although usually it is for specific days and times as identified on the care plan. Each person we work with has an individual support plan that they have agreed to, which highlights areas where support is needed. The amount of time spent with our support staff will vary according to individual need.

This is not a self-referral scheme, unless you are self funding, and you will need to have someone involved in your support, for example a social worker who can undertake an assessment of needs and secure funding.
A referral can be made through your Social Worker, Community Psychiatric Nurse or other professional agency / representative. Should you wish to self fund the support you can apply directly to Cardiff Mind.
Once the referral form is received and the support criteria are met, members of the Domiciliary Support Scheme may then arrange to visit and make an initial assessment of the support that is needed prior to the service provision commencing.

Complaints

Community Support
The Community Support service works in partnership with the council’s Floating Support Team, with funding from Supporting People, to support people who have tenancy related issues and other support needs that could have a detrimental effect on them maintaining a safe and secure home.
Aims
The aim of the service is to encourage people who have enduring mental health problems to develop the social and general living skills which will enable them to live successfully in their community.
The Cardiff Mind skilled and committed staff work with people to support them to identify their goals and draw up a goal-orientated individual support plan which can include, for example:
- Support can be provided to attend medical appointments and accessing medication
- Budgeting, maximising benefit claims, dealing with correspondence etc
- Accessing services which may promote development of social skills
- Accessing training, volunteering or employment opportunities
- Any tenancy related issues which have an impact on an individual’s housing and support needs
- Networking and advocacy issues
- Managing aspects of mental health difficulties
- Support to contact other services
Application and Referral
Application or identification of need can be made through one of the local hubs, who will refer on to the Council Floating Support Team. They will then allocate your case to us depending on capacity. We have 9 places allocated to us.
Acceptance
Once accepted onto the scheme the tenant will be allocated a support worker. The Support Worker will explain the process of support and in particular, they will explain the tools they will use to document support, assessment of needs and support plans.
Support
Support will take place for a duration of approximately 12 weeks and is free of charge.
Withdrawal of Support
The support scheme is not permanent and we will withdraw support over time in a planned way, in co-operation with the tenant, funders, and other agencies and sources of support where appropriate.
